Please refer to our brochure for all aspects of your treatment in this package. We treat professional athletes, week-end warriors, or folks suffering from Conditions
such as: sciatic pain, low back pain, frozen shoulders, rotator cuff injuries, sprained ankles, plantar fasciitis, etc.

Treating Weight Control
We use acupuncture and ear seeds. We also have you fill out a one-time diet diary. We send that through a Diet Analysis
computer program and go over the results with you, once it's printed. We conduct nutritional counseling with you. We will be partnering with Fitness 1 and help you with
your exercise program. |

Pricing for Packages
We print both Super Bills, HCFA-1500 or CMS1500 Forms for our patients to give to their insurance companies.
We have a medical biller on staff who will check to see if your insurance plan covers acupuncture.
Both the Weight Control and Sports Medicine "Packages" cost $95.00 per Treatment
